The application costs $25, non-refundable, and the city mails a renewal notice each year (the rates page gives $20). If you refuse an offer made before 15 March you forfeit your fees and your place; after 15 March you can refuse without penalty. The annual renewal fees, less a small service charge, are credited to your first season if you accept a slip.
