What waiting lists actually cost

What harbours, airports and clubs across the US ask for to put you on their list: the fee, how long you get to answer, how many times you can say no, and how they decide who is next.

Every line here comes from the place's own published rules, and every line says where we read it and when. If a number is not there, we say so rather than guessing: an index that fills in the gaps is not an index, it is a rumour with a table around it.
